Please bring the following required documents (Must be 65 and older OR fully disabled between the ages of 18 and 64):

  1. Social Security, SSI and SSP Income
  2. Any retirement, pension, annuity, and IRA distributions
  3. Any Interest or Dividend Income
  4. Any W2’s if currently working
  5. Driver’s License or State ID
  6. 2023 Paid Tax Receipts (County, Local, and School Taxes)

If a Renter, you need all the above AND:

  1. PA Rent Certificate
    (Please contact our office if you do not have one or if you have any questions)
