Who Can Qualify?

Age Criteria
65 and older
Widows and widowers 50 and older
People with disabilities 18 and older

Income Criteria
Household income must be $48,110 or less annually

What You’ll Need to Apply
To verify your eligibility, please bring copies of the following:

Proof of Age (any one of the following):
Birth certificate
Driver’s license or PA ID card
Medicare card
Passport
Military discharge papers (if age is shown)
PACE/PACENET card
Church baptismal record
Hospital birth record
Blue Cross/Blue Shield 65 Special Card
Naturalization/immigration papers (if age is shown)

Proof of Income
Social Security, SSI, and SSP
Railroad Retirement Tier 1 Benefits (if applicable)
Any Pension, Annuity, IRA Distributions, and Railroad Retirement Tier 2(if applicable)
Interests and Dividend Income
Any W-2s or other incomes from the year being claimed
Any Gambling and Lottery winnings, including PA Lottery winnings, and the value of other prizes from the year being claimed

Proof of Property Taxes or Rent Paid
Homeowners: Receipted tax bills for the year being claimed
Renters: Rent certificate completed by your landlord or notarized affidavit if landlord’s signature cannot be obtained
